Make a Report
To submit a concern or report an incident of Sexual Misconduct under Title IX, such as Sexual and Gender-based Harassment, Sexual Assault, Dating/Domestic Violence, Stalking, Sexual Coercion, Sexual Exploitation, Sexual Intimidation, and Retaliation, use the reporting form below.
Completion of this form does NOT begin any formal investigation under Title IX nor does it constitute a formal complaint under the Policy and Procedures on Sexual Harassment and Other Sexual Misconduct, but will allow the Title IX Coordinator to contact the individual concerned. The Title IX Coordinator will be able to offer supportive measures, provide information and answer questions about the grievance process, and assist members of the campus community to access appropriate resources.
(This form can be used by “Responsible Employees” to report any incident of sexual misconduct they are informed or aware of.)
